The patient, a 90 year-old man in good health, presented with left-sided hemiparesis secondary to a tandem occlusion of the right carotid artery at the level of the cervical segment and the M1 segment of the right middle cerebral artery. After several failed catheterization attempts distal to the proximal carotid occlusion using a 5F diagnostic catheter and 5F aspiration catheter, a #TigertrieverXL was deployed in the occluded segment allowing for distal catheterization and mechanical thrombectomy at the level of the middle cerebral artery.
